Provided the phone number for central registry 804-726-7567. Provider stated that she no longer accepts subsidy children, she will call and cancel her contract. There was no subsidy machine visible in the family day home. Subsidy # 804-726-7090. All background checks (fingerprinting, central registry and sworn disclosure) are good for 5 years from the date on the form.
An unannounced monitoring inspection was conducted today between the hours of 9:45 am through 11:00. There were a total of 5 children (17 points) with 2 caregivers. A total of 5 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: toy play, manipulatives, singing, bottle feeding and snack. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles are not labeled with the child's full name and dated, if more than one infant is in care. Evidence: Two bottles were labeled with the child's last name and one bottle was not labeled. None of the bottles are dated. Plan of Correction: We will have the bottles labeled and dated.
Standard #: 22VAC40-111-280 Description: Based on observation, potentially poisonous substances are not stored in areas inaccessible to children. Evidence: Cleaning chemicals were found in an unlock cabinet in the bathroom, these chemicals are accessible to the children. Plan of Correction: I will put a lock on the cabinet. The children are always supervised while in the bathroom.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the provider has not maintained a completed central registry for each assistant within 30 days of employment. Evidence: Assistant #1, date of hire 1/16/18 does not have a completed central registry on file. The Provider stated that she mailed it but it has not been returned. Plan of Correction: I called central registry and left a message. I will fill out a new form if they have not received it.
