Comments:
An unannounced renewal inspection was initiated today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today between 1:35-3pm. There were 7 children (plus the Provider's daughter), totaling 22 points, under the supervision of 2 caregivers. Today, the children participated in circle time, enjoyed free play, listening and danced to music and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken rice, leeks, carrots and milk. The Provider is currently certified in MAT (medication administration training). However, there are no children with any medication needs at this time. Standard #: 22VAC40-111-120-B
Description: Based on file review, 1 caregiver does not have all required information on file. Evidence 1. Caregiver #3 only has only one reference check on file.

Plan of Correction: The Provider will get a second reference letter or complete a reference check.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 1 out of 5 children's records does not have the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached Evidence 1. Child #4 only has one emergency contact on file.

Plan of Correction: The Provider will ask the parent to complete this today.
