54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced focused monitoring inspection was conducted from 1:30 ? 3:30 pm with the provider and her assistant. There were eight children in care, ranging from three months to three years of age, for a total of 24 points. Children were observed napping, playing with blocks, and eating afternoon snack. Snack served today is crackers and applesauce. Six children's records were reviewed. Standard #: 22VAC40-111-100-A Description: Based on a review of six children's records, not all children have documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #3 with a start date of 2/26/2018 did not have documentation of a physical examination. Plan of Correction: Will ask the parent for documentation of the physical.
Standard #: 22VAC40-111-60-B Description: Based on a review of six children's files, it was determined that not all files were complete. Evidence: The file for child #1 (date of attendance 2/12/18) did not have the mother's phone number, work address or work phone number, and did not have the father's work address or work phone number. Child #1's file was also missing the insurance name and policy number. The file for child #2 (date of attendance 3/5/18) was missing the mother's work address and work phone number and the father's phone number, work address and work phone number. The file for child #3 (date of attendance 2/26/18) was missing the mother's phone number, work address, medical insurance name and policy number, and the addresses for 2 emergency contacts and the phone number for one emergency contact. Plan of Correction: I will ask them to complete the information tonight.
Standard #: 22VAC40-111-90-A Description: Based on a review of six children's records, the provider did not obtain documentation that children have been adequately immunized in accordance with the Code of Virginia and applicable State Board of Health regulations. Evidence: Child #3 with a start date of 2/26/2018 did not have documentation that the child is adequately immunized. Plan of Correction: Will ask the parent for the immunization record.
