The Authorization form for Non-Prescription Topical Skin Products should be updated yearly.
An unannounced monitoring visit was conducted today between the hours of 12:52pm through 2:30pm with the provider on site. There were 9 children (27 points) with the provider and two assistants. A sample of 4 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: outside play and nap time. The provider has MAT training to administer medications. The provider and assistants are current in CPR and First Aid training. There were no medications on site today. Standard #: 22VAC40-111-780-B Description: Based on review of children's records , the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review yearly. Evidence: 1. Child A began with the provider on 2/13/2016. The last yearly review with the parent was signed on 1/03/2017. 2. Child B began with the provider on 11/09/2015. The last yearly review with the parent was signed on 12/08/2016. Plan of Correction: I will ask the parents to review the information and sign and date the form of the review.
Standard #: 22VAC40-111-920 Description: Based on interview, the provider stated she provides meals and snacks, but did not have a menu for the current one week period. Evidence: 1. The provider stated she provides meals and snacks for the children, but had not printed the menu out for the parents to view. Plan of Correction: I will print the menu out.
Standard #: 22VAC40-111-240-A Description: Based on observation, furnishings of the family day home outside were not maintained in a safe and operable condition. Evidence: 1. One green and beige child's riding car had a wheel with a large crack going through the tire and was taped around the tire. The crack was visible and not completely covered by the tape. Plan of Correction: I will throw the riding car away.
