- Technical Assistance:
We discussed requirements for children's and caregiver's records. The risk assessment for a violation was provided. We reviewed the requirements for caregiver records, training documents, spacing of openings at the bannister and below an interior wooden gate, and the posted emergency map.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:20 AM to 1:50 PM. Today, there were eleven children, ages infancy through four years (twenty-eight points) with three caregivers. I was accompanied by another licensing inspector from our office. I viewed activities, equipment, staffing, supervision, interactions with children, posted information, menu/nutrition, lunch routine, diapering procedures, emergency planning, emergency supplies and drill records, four records for caregivers/residents, five records for children, caregiver training, indoor and outdoor areas. The children enjoyed songs, stories, and a variety of age appropriate toys. The caregivers were observed providing comfort, guidance and frequent interactions with the children. Standard #: 22VAC40-111-120-B Description: Based on a review of four records for caregivers/residents, and interviews with caregivers, there was required information missing from the record of caregiver 2. Evidence: There was only one written reference in the file. The caregivers indicated that they had a second completed reference form and it could not be located. Plan of Correction: The provider will obtain a second written reference will be obtained for the record of caregiver 2. The provider will make sure all required information is maintained in the files.
Standard #: 22VAC40-111-60-B Description: Based on a review of five records for children, and an interview with the provider, there was information missing from the record of child 5. Evidence: The emergency contact section of the registration form did not include a second name, address and phone number. Plan of Correction: The provider immediately called the parent of child 5 and obtained the name, address and phone number of another emergency contact. In the future, the provider will review all paperwork at the time of enrollment and make sure all information was provided.
