Today an unannounced renewal visit was conducted with the Provider. This visit took place in the morning between 9:15am-12:45 there were 8 children with the Provider and 2 assistants for a total of 32 points within the supervision guidelines. The children were observed having morning free play, outdoor free play, lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-800-B
Description: Based on review of provider records, the provider did not review and update the emergency plan for 2018. Evidence: The emergency plan had not been reviewed and updated since 3/17.

Plan of Correction: Provider updated the emergency plan during this inspection.

Standard #: 22VAC40-111-120-A
Description: Based on review of records, provider not maintain a record for each caregiver. Evidence: Upon arrival, a new Caregiver(#2) was present, however Provider did not have record for the caregiver.

Plan of Correction: Provider stated that the caregiver just started, and is in the process of completing background checks and record.

Standard #: 22VAC40-111-120-B
Description: Based on review of two caregiver records, the provider did not maintain complete records. Evidence: Caregiver #2's record did not include name, address, verification of age, job title, date of employment, name and address of emergency contact person, documentation of two or more references were checked.

Plan of Correction: Provider stated that she will have them complete all of the paperwork for the record today.

Standard #: 22VAC40-111-170-A
Description: Based on documentation review, the provider did not obtain tuberculosis (TB) tests or screenings for each caregiver prior to coming in contact with children. Evidence: Caregiver #2 did not have documentation of Tb screening in the record.

Plan of Correction: Provider stated that they will have the tb done by the end of the week.

Standard #: 63.2-1720.1-B-3
Description: Based on review of records and interview with provider, the provider did not obtain a Criminal Record fingerprint check for a caregiver. Evidence: 1. Record for Caregiver #2 (hired 4/16/2018) did not have documentation of a Criminal Record fingerprint check completed prior to employment. 2. Provider confirmed that a fingerprint criminal record check have not been completed for caregiver #2.

Plan of Correction: Provider stated that the caregiver will make an appointment to have it done on Saturday.
