Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding updated FBI background checks.

Comments:
An unannounced renewal inspection was conducted today, April 25, 2018 to determine the provider's compliance with licensing standards. In attendance were 11 children totaling 28 points being supervised by the provider and assistant. Children were observed interacting with the provider and assistant, participating in organized activities, and playing with toys . Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home were inspected with no hazards found. The family day home had the required postings displayed on the wall upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. Five children's records and all provider records were inspected with the majority of the required information found. The following was also inspected: fire drill log, emergency preparedness plan, parent agreements, and other pertinent information. See attached notice for violations found on this day. Standard #: 22VAC40-111-960-E
Description: Based on the inspection of infant bottles, the required labels were not on the bottles. Evidence: During the inspection of infant bottles, two bottles were observed without the proper labels. Licensing standards require bottles to be labeled with the child's full name and the date.

Plan of Correction: Per provider: We will start labeling the bottles.

Standard #: 22VAC40-111-60-A
Description: Based on the inspection of children's records, one record was not updated.

Evidence:
During the inspection of five children's records, it was discovered child #3's record was last updated 3/02/2017.

Plan of Correction: Per provider: I will have the parent to review, update, and sign the application.

Standard #: 22VAC40-111-60-B
Description: Based on the inspection of children's records, the required information was not listed. Evidence: During the inspection of five children's records, it was discovered child #3, #4, and #5 did not have a second emergency contact listed. Licensing standards require two emergency contacts to be listed with the full address and telephone phone number for each contact.

Plan of Correction: Per provider: I will have the parents to update the record by adding a second emergency contact.
