Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding updated background check regulations.

Comments:
An unannounced monitoring inspection was conducted today, April 27, 2018 to determine the provider's compliance with licensing standards. The inspection was initiated at 2:25pm and concluded at approximately 3:50pm. In attendance were 6 children totaling 15 points being supervised by the provider. Children were observed interacting with the provider, watching a children's program, and playing with toys. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home used by children were inspected with no hazards found. The family day home had the required postings displayed on the bulletin board upon entering the porch. The first aid kit, fire extinguisher, and emergency supplies were inspected. The following was also inspected: children's records (4), provider records (2), evacuation log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. Standard #: 22VAC40-111-830-B
Description: Based on the inspection of administrative records, the required shelter-in-place drills were not conducted. Evidence: During the inspection of the shelter-in-place drills, it was discovered the family day home only practiced one shelter-in-place drill for 2017. Licensing standards require a minimum of two shelter-in-place drills to be practiced each year.

Plan of Correction: Per provider: I will ensure two shelter-in-place drills are conducted each year.

Standard #: 22VAC40-111-100-A
Description: Based on the inspection of children's records, the required documentation was not obtained. Evidence: During the inspection of four children's records, three of the children's records did not have a physical examination in the record. Licensing standards require providers to obtain documentation of a physical examination prior to child's attendance or within 30 days after the first day of attendance.

Plan of Correction: Per provider: I will talk with the parents to obtain a copy of the children's physical and place it in the record.
