An unannounced Subsidy Health and Safety inspection was conducted today, April 27, 2018 to determine the family day home's compliance with Subsidy Health and Safety vendor requirements. The inspection was initiated at 2:25pm and concluded at approximately 3:50pm. Administrative records, all areas in the home used by children, and indoor common areas were inspected. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDHOM-000-006-B Description: Based on the inspection of administrative records, the required allergy plan was not obtained. Evidence: During the inspection of administrative records, it was discovered child #1 with a diagnosed food allergy did not have an allergy plan in the record. Plan of Correction: Per provider: I will talk with the mother to obtain an allergy plan from the child's doctor.
