An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding updated background check regulations.
An unannounced monitoring inspection was conducted today, April 26, 2018 to determine the provider's compliance with licensing standards. The inspection was initiated at 1:50pm and concluded at approximately 3:25pm. In attendance were 12 children totaling 27 points being supervised by the provider and assistant. Children were observed sleeping on cots with sheets, playing with toys, and eating snack (consisting of apples, cheese crackers, and milk). Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home used by children were inspected with no hazards found. Due to previous rain, the outdoor playground in the rear yard could not be inspected.
The family day home had the required postings displayed on the bulletin board in the children's room. The first aid kit, fire extinguisher, and emergency supplies were inspected with the required items present. Five children's records and one provider's record were inspected with the required information found. The following was also inspected: fire drill log, shelter-in-place drills, parent agreements, and other pertinent information. See attached notice for violations found on this day.

Standard #: 22VAC40-111-180-A Description: Based on the inspection of administrative records, the required update was not obtained.
During the inspection of one provider's record, it was discovered the tuberculosis (TB) test dated 12/31/15 expired on 12/31/17. Licensing standards require an updated TB test or screening every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Plan of Correction: Per provider: I will have the staff to obtain an updated TB test.
