Comments:
An unannounced renewal inspection was conducted today from 10:15 am - 1:00 p.m. There were ten children present with the provider and two assistants for 32 points. I observed children playing outdoors, having the first picnic lunch of the year outdoors, coming in for bottles and milk, and playing with cars and trucks. I also observed singing and preparation for naptime to classical music. If you have any questions, please contact me at brenda.kuhlman@dss.virginia.gov

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation today, a medication was stored in an unlocked place. Evidence; A bottle of children's Tylenol was observed in a high drawer, but it was not in a locked area.

Plan of Correction: That may be from a family member since we don't give medications. It was removed.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records today, one record did not contain all required information. Evidence: Of 9 records reviewed today, there was no signed authorization by the parent for emergency medical treatment as required.

Plan of Correction: I will have her sign this today.

Standard #: 22VAC40-111-210-A
Description: Based on a review of staff records today, not all staff had documentation of at least 16 hours of required annual training. Evidence: The provider had 14 hours of training documented, and Assistant # 1 had 7 hours documented.

Plan of Correction: We are signed up for more training next month.

Standard #: 22VAC40-111-240-A
Description: Based on observation during today's inspection, some areas of the family day home were not maintained in a safe condition. Evidence: While children were present outdoors, there was an area of the yard where construction machinery, tools and wood boards were accessible in the yard, and a shed was not locked which contained construction supplies. There were also large holes in the ground where fence posts will be placed, but these posed a tripping hazard today.

Plan of Correction: We are putting up a fence this weekend so the construction supplies will fenced away from the children. They should finish it this weekend but we will put up barriers so children do not go near the holes for the fence posts. or anything else. The shed will be blocked by the fence.

In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal.
