Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and household members must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn. At the start of this unannounced monitoring inspection there were a total of 11 children (28 points) with 3 caregivers. A total of 11 children?s files, 3 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: large motor activities outside in the backyard, learning, music, diapering, bathroom breaks, hand washing, games, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-780-B Description: Based on review, 1 of 11 children's files reviewed indicated that annually, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review. Evidence - 1. On the date of inspection (05/02/2018), the documented annual review for Child #7 was not current. 2. The last review for Child #7 was listed as - 04/13/2017. Plan of Correction: The parent of Child #7 shall review, sign, and if needed make the necessary updates.
Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 11 children's files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to a child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (05/02/2018), documentation of a physical examination for Child #8 was not available for review. 2. The start date for Child #8 was listed as 08/19/2017. Child #8 was also in attendance during today's inspection. 3. This same standard was cited during an inspection conducted previously on 11/29/2017. Plan of Correction: The parents of Child #8 shall be contacted in order to obtain this documentation and place it on file.
Standard #: 22VAC40-111-120-B Description: Based on review, 1 of 3 staff files reviewed indicated that the documentation of two or more references (as to character and reputation as well as competency) were checked before the employment of all staff. Evidence - 1. On the date of inspection (05/02/2018), 0 of 2 documented references were available for review for Staff #3. 2. The Provider stated (during an interview conducted on the date of inspection) that Staff #3 started a couple of days ago (04/30/2018). Plan of Correction: At least 2 references shall be obtained and checked for Staff #3.
Standard #: 22VAC40-111-90-C Description: Based on review, 1 of 11 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (05/02/2018), the most current immunization documentation on file for Child #7 was dated - 10/06/2017. 2. On the date of inspection, based on the information on file, Child #7 was under the age of two years. Plan of Correction: An updated immunization page shall be obtained from the parent of Child #7 and placed on file.
Standard #: 63.2-1720.1-A Description: Based on review, 1 of 3 staff files reviewed indicated that not all employees underwent a fingerprint background check (in accordance with subsection B of this code) prior to their employment. Evidence - 1. The Provider stated (during an interview conducted on the date of inspection - 05/02/2018) that Staff #3 had started in the Family Day Home's (FDH) employment two days ago (04/30/2018). The Provider also confirmed that Staff #3 was currently being paid for her employment. 2. On the date of inspection, an "Eligibility Letter" for Staff #3 was not available for review. 3. The Provider stated (during the same interview) that she did not realize that this documentation needed to be obtained prior to Staff #3's employment. Plan of Correction: An appointment with Fieldprint of Virginia shall be scheduled today in order to obtain this documentation.
