54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was initiated today. The physical plant, 5 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 2-3:40pm. There were 8 children, totaling 28 points, under the supervision of 3 caregivers. During the inspection, children were observed enjoying free play, resting at nap time and being fed a bottle(infant) and eating afternoon snack. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken nuggets, fries, mixed fruit, yogurt, and milk. Two of the caregivers are currently certified in MAT (medication administration training). Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-111-830-A Description: Based on file review, the Provider did not have documentation of practicing an emergency evacuation drill monthly with children in care. Evidence 1. The evacuation drill log did not include a drill having been completed for the month of April 2018. Plan of Correction: This will be completed and documented.
Standard #: 22VAC40-111-120-B Description: Based on file review, 2 out of 3 assistants do not have all required information on file. Evidence 1. Caregiver/household members #2 and #3 do not have the following on file: Verification of age, Name address and telephone number of a person to be notified in an emergency, and documentation that two or more references as to character and reputation as well as competency were checked before employment began. Plan of Correction: The Provider will get this to keep on file.
Standard #: 22VAC40-111-60-A Description: Based on file review, 1 out of 5 children's records has not been updated annually. Evidence 1. Child #1's record was last updated on 1/23/17. Plan of Correction: The Provider will ask the parent to update the file.
Standard #: 22VAC40-111-280 Description: Based on observations, there was a hazardous substance which was not kept in an area inaccessible to the children. Evidence 1. There was a bottle of disinfectant spray hanging on the side of the diaper changing table. Plan of Correction: This will be moved.
