54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was completed on 5/7/2018. There were five children present with the provider and an assistant supervising. The point total was eleven. Five children's records were reviewed. The provider, household, and assistant's records were all reviewed. The inspector arrived at the provider's home at 10:30 A.M. and departed at 12:45 P.M. The children were observed during outside time, during free play time, flashcards, during lunch, and during nap.
Standard #: 22VAC40-111-90-A Description: Based on review of five children's records the licensee failed to ensure that all immunization were obtain according to the requirements of standard. Evidence: 1. The record of child # 3 had a parent's record for the immunization required. The standard required that the immunization are documented from a physician or a physician designee. The start date for child # 3 was 12/15/2017. Plan of Correction: The licensee will obtain an immunization record from the physician as soon as possible.
