An unannounced inspection was conducted today from 200-315. There were 6 children (one child was Provider's own grand child) with the Provider for a total of 12 points. The Provider has attended various trainings and has current First Aid/CPR. The Provider and household members are current in background checks and finger prints have all been completed. The Inspector also checked 4 children's files and evacuation procedures. Hand washing and potty time was also observed. Please call with questions 703-479-4679
Standard #: 22VAC40-111-70-A Description: Based on record review, the Provider did not get parent's liability insurance acknowledgement for child # 4. Evidence: 1. Child # 4's file has the liability insurance form but it does not say whether the provider has or does not have liability and it is not signed by the parent. Plan of Correction: I will ask the parent to sign it.
Standard #: 22VAC40-111-90-A Description: Based on record review, the Provider did not obtain documentation of immunization for child #3. Evidence: 1. Child # 3 began on Saturday and does not have an immunization record maintained in the file. Plan of Correction: I will request the information
Standard #: 22VAC40-111-280 Description: Based on observation, cleaning supplies, deodorizer, and disinfectants were not locked and inaccessible to children. Evidence: 1. The bathroom cabinet lock was not secure and the gate to the kitchen was not securely fastened to prevent access. The kitchen cabinet and the bathroom cabinet has cleaning items and disinfectants stored in these areas. Plan of Correction: I will lock it up
