Description: Based on review of caregiver records, caregiver did not obtain 16 clock hours of training annually. Evidence: Caregiver #1 had 13 hours of training and need 3 more hours.

Plan of Correction: Provider stated that due to weather some of the trainings were canceled. Provider stated that she is scheduled to take a training on 5/15/2018.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not have children wash hands with soap and running water before and after eating meals or snacks. Evidence: Provider were observed feeding 3 children lunch however two of the children hands were not washed.

Plan of Correction: Provider stated that she she used a wipe for one of the children but will make sure all of their hands are washed. Provider washed all hands.
