Technical Assistance:
Today we discussed posting the menu and background checks.

Comments:
An unannounced monitoring inspection was conducted today between 12noon and 2pm. There were five children in care with the provider and an assistant. Children present total points equaled 18. Children and household records were reviewed. Children were observed in provider and self-directed activities. The following violations were identified and appear on the violation notice; children's records and background checks.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review and interview the provider failed to ensure that each chid's record contains all required information. Evidence: 1 - The record for child 2 and 3 do not contain documentation of annual information update. a. Child 2 has a documented first date of attendance as 9/6/16 b. Child 3 has a documented first date of attendance as 5/16/16. 2 - The provider confirmed that there is no documentation annual update in the records of children 2 and 3.

Plan of Correction: The provider will obtain the information from the parents as soon as possible.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview the provider failed to obtain an immunization before a child may attend. Evidence: 1 - The record for child 1 does not contain any documentation of immunizations. a. Child 1 has a documented first date attendance as 3/6/18. 2 - The provider confirmed that there is not any documentation of immunization in the record of child 1.

Plan of Correction: The provider will obtain the information from the parent as soon as possible.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review and interview the provider failed to ensure that persons living in the home has a current sworn statement or affirmation. Evidence: 1 - The record for the provider contains a sworn statement dated 3/10/15. a. The provider confirmed that the sworn statement is outdated. 2 - The record for household member 1 contains a sworn statement that is date 3/10 15. b. The provider confirmed that the sworn statement for household member 1 is outdated.

Plan of Correction: The provider will obtain the required document from the department web site and place them in the records within 10 days of receiving this violation notice. Copies of the documents will be forwarded to the inspector within 10 of this inspection.
