Technical Assistance:
A caregiver with Medication Administration Training (MAT) certification must always be present when emergency medications are on site.

An unannounced monitoring inspection was conducted from 2:30 ? 5:15pm with the substitute provider, an assistant and the provider. There were nine children in care, ranging from 19 months to five years of age, for a total of 23 points. Children were observed napping, watching an age appropriate television program/video, having snack and playing outdoors. Snack served today was oranges, ginger snaps and water. Four children's records and four caregiver/household member records were reviewed. Emergency drills were reviewed and are in compliance. Standard #: 22VAC40-111-100-A
Description: Based on a review of four children's records, not all contained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child A's (date of enrollment: 8/24/2015) record did not have documentation of a physical examination.

Plan of Correction: Will ask the parents for a copy of the physical at pickup tonight.

Standard #: 22VAC40-111-220-A
Description: Based on interview with the substitute provider, a caregiver that had satisfactorily completed training to administer medication was not always on site. Evidence: There was no documentation that caregiver A or caregiver B had Medication Administration Training (MAT) certification and there was one emergency medication on site.

Plan of Correction: Caregiver A had MAT training and we will obtain a copy of the MAT certification documentation for the record.

Standard #: 63.2-1720.1-A
Description: Based on review of four staff/household member files, and an interview with the provider, it was determined that not all staff had criminal history record checks prior to their first day of employment. Evidence: Staff B (date of hire: 4/10/2018) did not have documentation of a criminal history record check in their file.

Plan of Correction: Employee made an appointment for fingerprinting during the inspection. Employee will not return to work until fingerprint background check is received.
