Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and household members must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. The Provider stated that she has already started this process.

Comments:
The following inspection took place between the hours of 9:30 am through 12 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling, Virginia. During this unannounced monitoring inspection there were a total of 8 children (22 points) with 2 caregivers. A total of 5 children?s files, 2 staff files, and 1 household member?s files were reviewed. The children were engaged in the following: toy play, music, free play, outside time on FDH playground, diaper changing, hand washing, lunch, nap time, and other organized activities. Standard #: 22VAC40-111-60-A
Description: Based on observation and interview, the Provider has not maintain an up-to-date record at the Family Day Home (FDH) for each enrolled child. Evidence - 1. On the date of inspection (05/10/2018), a children's record for Child #8 was not available for review. 2. The Provider stated (during an interview conducted on the date of inspection) that Child #8 started two weeks ago and that the parent has yet to return all of the required documentation.

Plan of Correction: The Parent of Child #8 shall be contacted today in order to obtain all of the required documentation.

Standard #: 22VAC40-111-90-C
Description: Based on review 3 of 5 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a children (who were not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (05/10/2018), three children who were under the age of two years, did not have immunization records conducted within the past 6 months. 2. The children and the dates of their immunization pages were: Child #2 (Immunization page dated - 09/07/2017), Child #5 (Immunization page dated - 09/07/2017), and Child #7 (Immunization page dated - 11/06/2017).

Plan of Correction: All applicable parents shall be contacted and current documentation shall be obtained.

Standard #: 22VAC40-111-140-B-3
Description: Based on review, the Provider was not currently certified in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care. Evidence - 1. On the date of inspection (05/10/2018), the most recent CPR certification on file for the Provider expired on 02/27/2018. 2. The Provider confirmed this finding on the date of inspection. 3. Currently, Staff #2 has CPR certification, as appropriate to the age of the children in care.

Plan of Correction: A class has been scheduled in order to renew this certification.

Standard #: 22VAC40-111-140-B-4
Description: Based on review, the Provider was not currently certified in first aid. Evidence - 1. On the date of inspection (05/10/2018), the most recent first aid certification on file for the Provider expired on 02/27/2018. 2. The Provider confirmed this finding on the date of inspection. 3. Currently, Staff #2 has first aid certification.

Plan of Correction: A class has been scheduled in order to renew this certification.
