Discussion of the subsidy regulations and how they apply to the Family Day Home (FDH).

Comments:
A Subsidy Health and Safety (SHSI) inspection was conducted at the Family Day Home (FDH) of Lucy Ayala's, located in Sterling, Virginia. This inspection was conducted on 05/10/2018, between the hours of 9:30 am through 12 pm to review supplemental health and safety requirements. An Unannounced Monitoring Inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. If you should have any future questions, please call. Description: Based on review, the Lockdown procedures were not practiced in the past year. Evidence - 1. On the date of inspection (05/10/2018), a review of the drill log indicated that there has not been a lockdown drill conducted in the past year. 2. The Provider confirmed this finding on the date of inspection.

Plan of Correction: A lockdown drill shall be practiced at least once a year (in addition to the 12 times or once a month fire drills, and the twice a year shelter-in-place drills).
