Technical Assistance:
Licensing inspector discussed fingerprinting requirements with provider.

Comments:
An unannounced monitoring inspection was conducted today from approximately 12:57pm-2pm. There were 7 children in care (18 points) directly supervised by 2 caregivers. A sample of 4 children?s records, 2 caregiver's records and 3 household member's records were reviewed. The children were observed napping and during feeding procedures. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-111-800-B
Description: Based on interview, the emergency plan was not updated at least annually. Evidence: The provider stated that she has thrown away the emergency plan and needs to write another one.

Plan of Correction: I will fill out the emergency plan.

Standard #: 22VAC40-111-830-B
Description: Based on record review, shelter-in-place procedures were not practiced a minimum of twice per year. Evidence: The last shelter-in-place procedure was practiced 11/1/2016.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the children's records reviewed were incomplete. Evidence: 2 of 4 records reviewed were missing required information. 1) Child A was missing an annual update. The file was last updated 8/2/2015. 2) Child B was missing 1 of 2 required emergency contacts and the mother's work phone number.

Plan of Correction: Child A is updated he had a new page because he moved. Here is a copy of it.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent violation notice was not posted. Evidence: The violation notice posted was from a visit conducted in 2016. At the time of inspection the most recent visit was conducted 10/2017.

Plan of Correction: I will post this visit now.
