Today an unannounced renewal visit was conducted with the Provider. This visit took place in the morning between 9:45am-12 there were 6 children with the provider for a total of 12 points within the supervision guidelines. The children were observed having morning free play, working on puzzles, fine arts, language and literacy, and lunch. Lunch consisted of Fish fillets, whole wheat bread, green beans, oranges and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider/household records, fire drill log, training hours, and emergency procedures were observed. Standard #: 22VAC40-111-60-B Description: Based on review of six children?s records, the provider did not maintain complete children?s records. Evidence: Child #1 & #2's record did not include documentation of the father's employers address and phone number and one of the designated emergency contact person's complete address. Plan of Correction: Provider stated that she will ask the parents today at pickup for the addresses.
