The schedule for current individuals is based on the results date from your last completed name based criminal history. Fingerprint Background Checks must be completed for all current staff and household members according to the following schedule: July ? December 2016 completed by June 1, 2018 January ? June 2017 completed by July 1, 2018 July ? December 2017 completed by August 1, 2018 January 2018 completed by September 1, 2018 Establish a separate file for each staff and household member

The Licensing Inspector for the Family Day Home of April Caudle, conducted an unannounced Monitor Inspection on May 22, 2018. There were six (6) children present under the direct care and guidance of three (3) caregivers. Children present were of various ages from 14 months - 5 years old; adult-to-child ratios were met. Children & Staff/Household records were reviewed. Provider continues to participate in the USDA Child Care Food Program, does not provide transportation and does not administer medication. An exit interview including review of the Risk Assessment & areas of non compliance found during today?s inspection were reviewed with the Provider on May 22, 2018. Inspection concluded at 1:40 PM.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the Licensee failed to have required information in each child record. EVIDENCE: Licensing Inspector reviewed five (5) children records. Record for Child #1 did not include the date of enrollment (1st day of attendance), documentation of the child's proof of birth/identity & emergency medical authorization signed by the child's parent. Record for child #5 did not include the complete address of two designated persons to contact in case of an emergency if the parent cannot be reached and the policy number of the child's medical insurance.

Plan of Correction: Will request documentation/information/signatures from the parent's of each child.

Standard #: 22VAC40-111-320-G
Description: Based on inspection & discussion with the Provider, the Licensee failed to have fireplace and associated chimneys inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. EVIDENCE: Fireplace and associated chimney locate on the ground floor has not been inspected annually. Most recent inspection completed on 10/30/2014.

Plan of Correction: Provider will have fireplace and chimney inspected.

Standard #: 22VAC40-111-350-E
Description: Based on inspection, the Provider failed to maintain hot water temperature in faucets used by the children in care at or below 120 degrees F. EVIDENCE: The hot water temperature in bathroom sink used by the children in care registered 135 degrees F.

Plan of Correction: Provider will reduce hot water temperature.
