An unannoucned monitoring inspection was conducted May 14 2018 between 11:15am and 12:30pm. There were 12 children in the direct care of 3 staff members. Children were observed playing outside on the play equipment. Children were also observed eating lunch, washing hands, being diapered, and napping. The day care is clean, organized, and offers plenty of space for children to utilize. The day care space also has plenty of age appropriate materials. Standard #: 22VAC40-111-80-A Description: Based on record review, 1 of 4 children's records reviewed did not contain proof of age and identity within seven business days of the child's first date of attendance. Evidence: Child #3 record, enrolled 2/5/2018, does not have complete proof of identity documentation. Plan of Correction: I will ask the parents for the paperwork.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation that a child has been adequately immunized prior to a child being able to attend the family day home. Evidence: Child #3 began care 2/5/2018 and the record does not contain documentation of an immunization record. Plan of Correction: I will ask the parents for the paperwork.
