Technical Assistance:
Discussed diaper creams and returning the creams to the parents if provider is no longer using the creams.

Comments:
An unannounced monitoring visit was conducted today between the hours of 9:48am through 12:00pm with the provider on site. There were 12 children (33 points) with the provider and 2 assistants. A sample of 5 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: singing songs at the table, outside play, snack, bathroom breaks, hand washing, and other organized activities. Infant care was also observed. The outdoor area is spacious and the inside area is clean and organized. The caregivers are certified in CPR and First Aid training. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-830-A
Description: Based on review, the provider failed to ensure that the emergency evacuation procedures were practiced monthly. Evidence: 1. The provider did not do a monthly emergency evacuation drill for November 2017 or March 2018.

Plan of Correction: I will make sure it gets done and document the drills.

Standard #: 22VAC40-111-240-A
Description: Based on observation, equipment of the family day home outside was not maintained in a safe, and operable condition. Evidence: 1. An orange and white riding car had a metal piece protruding out near where the child enters the play car and the wheels were rusted and bent on the frame of the car. 2. An orange and white riding car had one bent wheel on the frame of the car. 3. An orange and yellow riding car had rust around the wheel and was bent on the frame. 4. An orange and yellow riding car had rust around the wheel. 5. A pretend learning shopping cart had a green plastic bar at the bottom of the cart and had a large crack in the middle of the bar. 6. A little tikes mower was cracked at the base and had jagged edges.

Plan of Correction: I threw them out immediately and will replace them.
