Provider and inspector discussed the provider's upcoming move to a new location.

Comments:
An unannounced monitoring inspection was conducted today from 10:30am-11:30am. There were 2 children (4 points) directly supervised by 1 staff. The home, 1 staff records, 2 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed doing stamp art, reviewing letters, lacing letters and practicing fine motor skills. There was a supply of books, toys, and materials for the children. Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not document annual review of the emergency plan. Evidence: The emergency plan does not have documentation that it was reviewed annual. The last documented review was conducted in May 2016. Upon interview, the provider stated that a review of the plan was conducted but not written down.

Plan of Correction: I will document the review going forward.

Standard #: 22VAC40-111-60-B
Description: Based on two records reviewed, the provider did not ensure all required documents were in the child's file. Evidence: Child A does not have that a written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.

Plan of Correction: I will have the parent sign the document this afternoon.

Standard #: 22VAC40-111-180-A
Description: Based on documentation review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years. Evidence: Household member A has a tuberculosis screening which was conducted on May 9, 2016.

Plan of Correction: She is moving out at the end of the month.
