An unannounced monitoring inspection was conducted today from approximately 9:40am-12:15pm. There were 5 children in care (13 points) directly supervised by 2 caregivers. A sample of 5 children?s and 2 caregiver's records were reviewed. The children were observed during diapering procedures, playing with toys, counting, and engaged with caregivers. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Description: Based on record review, the provider did not review with the parent the emergency contact information to ensure the information is correct. Evidence: 4 of 5 records reviewed were not annually reviewed. 1) Child A - file was last reviewed 1/26/2017 2) Child B - file was last reviewed 1/26/2016 2) Child C - file was last reviewed 1/26/2017 4) Child D - file was last reviewed 1/26/2017

Description: Based on observation, the emergency plan was not reviewed at least annually. Evidence: The emergency plan was last reviewed 2010.

Description: Based on record review and interview with cargivers, the file of the assistant was incomplete. Evidence: The provider and assistant stated that the assistant was hired June of 2017 and the following information was missing from the file. 1) documentation of orientation. 2) The hire date, and an emergency contact

Description: Based on record review, children's records were incomplete. Evidence: 3 of 5 records reviewed were missing required information. 1) Child A was missing the addresses of 2 of 2 required emergency contacts. 2) Child B was missing the work phone numbers of both parents. 3) Child C was missing father's employment information.

Description: Based on record review, the family day home did not obtain documentation of additional immunizations. Evidence: The documentation of the most recent immunization record for Child B was dated 12/18/2015.

Description: Based on observation, unused outlets did not contain protective covers. Evidence: Two outlets located in the bathroom near the sink were not covered with protective covers.

Description: Based on observation, the landline telephone is one that operates with electricity. Evidence: The landline telephone is a cordless phone which requires electricity to operate.

Plan of Correction: I will get the phone by this week.

Description: Based on observation, the diapering surface was not wiped after each use. Evidence: After diapering a child, Caregiver A had not wiped down the surface of the changing mat. Caregiver A was reminded to wipe the surface by licensing inspector after she initiated another diaper change.

Plan of Correction: I will tell her to wipe down the surface today.

Standard #: 22VAC40-111-690-E
Description: Based on observation, diapers were not disposed in a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. Evidence: After changing the diaper of a child, Caregiver A disposed the diaper in a outside in the back yard. Multiple surfaces including the door knob and the top of another trash can were touched in the process of diaper disposal.

Plan of Correction: I will fix the foot operated trash can.

Standard #: 22VAC40-191-40-C-1-b
Description: Based on record review, a central registry search was not completed for a caregiver whom is involved in the day to day operations of the facility. Evidence: Caregiver A hired June/2017 did not have the results of a complete Central Registry Search on file. The Central Registry Search was initially conducted June 2017, it was sent back with a request of additional information. The additional information was not provided for the completion of the search.

Plan of Correction: I will give her the paper today and I will send you a copy.
