An unannounced monitoring inspection was conducted on 6/1/18 from 10:00am - 11:10am. At the time of the inspection there were eight children in care, ages one year old to five years old (18 points), with two caregivers present. Records were reviewed for five children and three caregivers/household members. Children were observed playing with outside, playing inside, and eating lunch. There was no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice and were discussed with the provider during the exit interview.
Standard #: 22VAC40-111-770 Description: Based on observation and interview, it was determined that the provider did not ensure that a working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times. Evidence: 1. There was no working portable battery-operated weather band radio available to caregivers during the inspection. 2. Caregiver #1 confirmed that she did not currently have a working portable battery-operated weather band radio Plan of Correction: The provider responded: A new working portable battery-operated weather band radio will be purchased.
Standard #: 22VAC40-111-500-C Description: Based on observation it was determined that the provider did not ensure that stationary outdoor playground equipment shall not be installed over concrete, asphalt, or any other hard surface. Evidence: 1. The Licensing Inspector observed a Little Tykes slide positioned on the concrete drive way with a child on it. 2. Caregiver #1 confirmed that there was a stationary outdoor playground equipment being used on concrete. Plan of Correction: The facility responded: Corrected during the inspection. It was moved on the concrete because the ground where it is usually located was muddy from all the rain. Going forward all stationary playground equipment will not be used on the concrete driveway.
