Discussion was held on the topics of current enrollment, current staffing, and training.
The following inspection took place between the hours of 8:30 am through 10:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced renewal inspection there were a total of 2 children (6 points) with 1 caregiver. A total of 3 children?s files, 1 staff file, and 4 household member?s files were reviewed. Standard #: 22VAC40-111-250-C Description: Based on observation, a child (five months in age) was observed asleep in a crib with a toy mobile attached to its side. Evidence - 1. On the date of inspection (06/06/2018), Child #3 was observed asleep in a crib that had a toy mobile attached to its side. 2. Based on the information on file, Child #3 was five months in age. Plan of Correction: The mobile shall be moved and/or taken down.
