54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted with provider. There were six children, ranging in age from 4 months to 3 years, present totalling 15 points. The children were dancing to Noodle on TV and preparing for rest. Three household files and one assistant file were reviewed. Six children files were reviewed - please see Supplemental Summary for missing items. Provider does not administer medication - Medication Administration Training is required to administer medication to day care children. Provider's first aid and CPR certification was renewed 4/30/2018. Standard #: 22VAC40-111-900-B Description: Based on observation, the provider failed to prevent the children from walking around while eating. Evidence: the provider gave each of the children a few french fries that they ate while walking around. Plan of Correction: Provider will have the children sit before giving them something to eat.
Standard #: 22VAC40-111-60-B Description: Based on review of six children's files, the provider failed to have all of the required information in each child's file. Evidence: Child 3 did not have documentation of written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. Child 1 and Child 3 did not have documentation of the new home address documented in the file. Plan of Correction: Parent will request information from the parents.
Standard #: 22VAC40-111-180-A Description: Based on review of three household files and one assistant file, the provider failed to have documentation of a repeat tuberculosis test or screening every two years for each individual. Evidence: Household members 1 and 3 did not have documentation of a current tuberculosis test or screening (expired 6/1/2018). Plan of Correction: Household members will get an updated TB test or screening.
