Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding injury reporting and various licensing standards.

An initial inspection was conducted today, May 30, 2018 to determine the provider's compliance with licensing standards for possible licensure. The inspection was initiated at 11:25 am and concluded at approximately 2:00pm. In attendance were three children totaling eight points being supervised by the provider. Children were observed participating in free play, using the restroom, and eating lunch. Today's lunch consisted of a sunflower butter sandwich on wheat bread, steamed broccoli, applesauce, and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home and front yard used for outdoor play were inspected with no hazards found.

The family day home had the required postings displayed. The first aid kit, fire extinguisher, and emergency supplies were inspected. Four children's records and all caregiver records were inspected with the majority of the required information found. See attached violation notice. The following was also inspected on this day: fire/shelter-in-place log, parent agreements, daily attendance log, daily schedule, information for parents, and other pertinent information.

Standard #: 22VAC40-111-770
Description: Based on the inspection of emergency supplies, the required item was not present. Evidence: During the inspection of the family day home's emergency supplies, it was discovered the family day home did not have a portable battery operated weatherband radio.

Plan of Correction: Per provider: I will purchase a weatherband radio within the week.

Standard #: 22VAC40-111-100-A
Description: Based on the inspection of administrative records, the required documentation was not available to review. Evidence: During the inspection of four children's records, it was discovered all of the children's records reviewed did not contain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Plan of Correction: Per provider: I will request and obtain the required documentation from parents.

Standard #: 22VAC40-111-60-B
Description: Based on the inspection of administrative records, the required information was not listed.

During the inspection of four children's records, it was discovered the following information was not listed:
1. Child #1 did not have a second emergency contact listed.
2. Child #3's second emergency contact did not have the street address listed.

Plan of Correction: Per provider: I will request and obtain the information from parents today.
