22VAC40-191 Background Checks for Child Welfare Agencies
An unannounced monitoring inspection was conducted today from 9:15am-12:15am. There were 10 children (31 points) supervised by 3 direct care staff. The home, 2 caregiver records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing outside with cars and pretend horses and transitioning to morning nap. Bottle feedings were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Standard #: 22VAC40-111-800-A Description: Based on record review, the family day home did not have a written emergency preparedness and response plan. Evidence: During the time of inspection, there was no documentation of a written emergency preparedness and response plan on site. Plan of Correction: I will check with my husband and we will find it.
Standard #: 22VAC40-111-60-B Description: Based on record review, 4 of 5 children's records did not contain all of the required information. Evidence: 1. Child #1 (start date: 1/3/2017), Child #2 (start date: 9/12/2016), and Child #3's (start date: 3/6/2017) records did not contain documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. 2. Child #5's (start date: 10/10/2017) record did not contain documentation of an address for the second emergency contact person. Plan of Correction: I will get the information this week.
Standard #: 22VAC40-111-180-A Description: Based on record review, 1 of 2 caregiver records did not contain documentation of a current report of tuberculosis screening form every two years from the date of the first screening. Evidence: Caregiver B's record's contained documentation of a report of tuberculosis screening completed on 6/6/2016 (expired 6/6/2018). Plan of Correction: I will get it next week.
Standard #: 22VAC40-111-240-A Description: Based on observation, all areas and furnishings of the family day home were not maintained in a clean, safe, and operable condition. Evidence: 1. The wooden diaper changing table, and areas of the wall, inside the back playroom have chipped and peeling paint. 2. A plastic playhouse in the outdoor play area was collapsed on the ground in the middle of the play yard. Plan of Correction: I will fix it.
Standard #: 22VAC40-111-340-C Description: Based on observation, the bathroom did not contain all of the required materials. Evidence: The children's bathroom did not contain toilet tissue. Plan of Correction: Fixed during time of inspection.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface was not nonabsorbent, as required. Evidence: The diaper changing mat was ripped on one corner. Plan of Correction: I will change it this week.
Standard #: 22VAC40-191-40-D-5-a Description: Based on record review, 1 of 2 household members' records did not contain a child protective services central registry check within 30 days of becoming 14 years old. Evidence: Household member B (date of birth: 5/10/2002)'s record did not contain documentation of a child protective services central registry check. Plan of Correction: I will do it next week.
