Background checks are now valid for 5 years from the date originally conducted. Any potential new staff person must have fingerprinting conducted before they can be hired.
An unannounced monitoring inspection was conducted with the provider and assistant. There were 10 children present, ranging in age from 1 year to 4 years, totaling 24 points. The children were having lunch and preparing for rest. Lunch served today: pizza, corn, and milk. Five children's files and three household/staff files were reviewed - please see Supplemental Summary for missing items. Provider does not administer medication - Medication Administration Training is required to administer medicine. Providers' first aid and CPR is valid until 12/10/2018. Standard #: 22VAC40-111-100-A Description: Based on review of five children's files, the provider failed to obtain documentation of a physical examination for each child within 30 days after the first day of attendance. Evidence: Child A (date of enrollment 4/30/2018) did not have documentation of a physical on file. Plan of Correction: Provider will request the information from the parent again.
Standard #: 22VAC40-111-60-A Description: Based on review of five children's files, the provider failed to have documentation of a record for each enrolled child. Evidence: there was no file for Child B available. Plan of Correction: Corrected during inspection: Child B was previously residential and did not require a file. Child B's parent completed the paperwork during today's inspection.
Standard #: 22VAC40-111-170-A Description: Based on review of three household files, the provider failed to have documentation of a tuberculosis test or screening for each adult household member. Evidence: Household member 3 did not have documentation of a tuberculosis test or screening on file. Plan of Correction: Household member 3 will get a tuberculosis test or screening.
Standard #: 22VAC40-191-40-D-4-a Description: Based on review of three household files, the provider failed to have each household member complete a sworn disclosure statement when a person in the home turned 18 years old. Evidence: Household member 3 did not complete a sworn disclosure statement after turning 18 years old. Plan of Correction: Corrected during today's inspection: Household member 3 completed a sworn disclosure statement during today's inspection.
