An unannounced monitoring inspection was conducted today from 2:30pm through 5:45pm. There were ten children present with the provider and one assistant at the onset of the inspection. Children, staff and household records were reviewed. Children were observed provider and self-directed activities. Emergency supplies were reviewed. Medication was reviewed. Violations regarding small appliances, physical environment, medication, and capacity were identified and appear on the violation notice.
Standard #: 22VAC40-111-40-A Description: Based on observation and interview the provider failed to ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence: 1 - Upon arrival to the family day home at 2:30pm there were 10 children present. However, the licensed maximum capacity is for nine. 2 - The provider confirmed that there were 10 children in care at the on set of the inspection. Plan of Correction: The provider will ensure that the licensed capacity is closely followed from now on.
Standard #: 22VAC40-111-60-B Description: Based on medication review the provider failed to ensure that written authorization is obtained if a caregiver is to administer prescription medication to the child. Evidence: 1 - The family day home provider has a prescription medication for child 1, an Epi Pen and inhaler. 2 - The provider stated that if child 1 were to have a need for the medication she would administer it to the child. Plan of Correction: The provider will obtain the required written authorization from the parent as soon as possible.
Standard #: 22VAC40-111-240-A Description: Based on observation and interview the provider failed to ensure that areas are maintained in safe condition. Evidence: 1 - There is a top loading freezer accessible in a room used for children in care. There is not a child proof latch on the freezer. a. While the room used for sleeping infants there were four school aged children present for care today who could easily access the top loading freezer. 2 - The provider confirmed that the top loading freezer was accessible to the school age children in care. Additionally, the provider agreed that the freezer does not have a child proof latch. Plan of Correction: A child proof latch will be installed on the top loading freezer or the freezer will be moved to a location that is not accessible to children in care.
Standard #: 22VAC40-111-320-C Description: Based on observation and interview the provider failed to ensure that no electrical device accessible to children will be placed so that it could be plugged into an electrical outlet while in contact with a water source, such as a sink, Evidence: 1 - In the bathroom used by children in care there is a scent diffuser on a shelf next to the sink. a. The device is plugged into the outlet next to the sink. 2 - Children were observed using the bathroom during the inspection. 3 - The provider confirmed that the device is plugged in and next to the sink in the bathroom. Plan of Correction: The provider will remove the device as soon as possible.
