Technical Assistance:
Discussed with provider background checks for adult household members and members that are about to reach the age of 14; Discussed land line telephone, outside play area and permission forms for swimming activities if the provider decides to use the community pool.

Comments:
An announced non-mandated monitoring inspection was conducted today from 11:40 am to 1:30pm. There were no children in care. The purpose of this inspection was to inspect the provider's new physical space as she has changed residence. Updated emergency preparedness plans and evacuation maps were available for review. Emergency supplies were maintained. A walk through of the house was conducted which included the area that will be used for the day care, backyard and the bathroom designated for children's use. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on record review, provider did not maintain a current subsequent Tuberculosis (TB) Screening for an adult household member every two years. Evidence: Household member # 1's most recent TB screening expired on 1/27/18.

Plan of Correction: I will get a new TB screening done for household member.

Standard #: 22VAC40-191-60-C-1
Description: Based on staff interview, family day home does not have the original criminal history record report for an adult household member. Evidence: 1. Household member # 1's most recent criminal record check (CRC) expired in May 2017. 2. Staff # 1 stated that she did not renew the CRC for household member because he works in Florida. Staff # 1 stated that she thought he would not need it. 3. Staff # 1stated that household member # 1 comes and lives in the house on the weekends. Staff # 1 provides care for children on the weekends as well.

Plan of Correction: I will have fingerprinting done for household member # 1.
