Comments:
A Subsidy Health and Safety inspection was conducted on April 24, 2018 to review supplemental health and safety requirements. Upon arrival in the family day home, two children were in care. The Provider and an adult household member were present during the inspection. Based on the ages of the children, the point total was 4. After school, one additional child arrived by school bus, this increased the point total to 5. The children were served an afternoon snack and played. The first aid kit was observed in the fdh. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on documentation reviewed, the Provider's written lock down procedures did not address all procedural components. Procedural components are to include: methods to alert caregivers and emergency responders; methods of communication with parents and emergency responders; procedure to reunite children with a parent or authorized person designated by the parent to pick up the child. Caregiver training requirements, drill frequency, and plan review and update; and other special procedures developed with local authorities.

Plan of Correction: The Provider and the Assistant will review the subsidy requirements and will address all procedural components on a written lock down emergency plan.
