The licensing inspector conducted an unannounced monitoring inspection on 06/18/2018 from 11:50am to 1:58pm. The children were napping upon the inspector?s arrival. Today, there were eight children, totaling 17 points, present with three caregivers. Interviews were held with the caregivers throughout the inspection. The provider does administer medication; however, they did not have any being given at this time. The provider does not provide transportation. The provider does have a pet. Five children?s records, one caregiver?s record and two caregiver/household member records were reviewed during the inspection. Last emergency drill: 05/14/2018 Last shelter-in-place drill: 05/22/2018 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-940-A Description: Based on observation, the facility did not ensure that the refrigerator temperature was maintained at or below 40?F. Evidence: The thermometer in the facility's refrigerator read 48?F. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-111-100-A Description: Based on record review, the center did not ensure that one of five children?s records contained documentation of the child's physical examination prior to the child's attendance or within thirty days. Evidence: The record of child #3, enrolled 01/8/2018, had documentation of a physical examination dated by a physician on 02/16/2018. Plan of Correction: Will make sure I get children's physical records within 30 days from now on.
Standard #: 22VAC40-111-60-B Description: Based on record review, the facility did not ensure that two of five children's records contained all required information. Evidence: 1. Child #2's (start date: 12/12/16) record did not contain the address of an emergency contact. 2. Child #4's (start date: 6/07/17) record did not contain the policy number of the child?s medical insurance. Plan of Correction: Will get the information from the parents by Friday.
Standard #: 22VAC40-111-280 Description: Based on observation, the facility did not ensure that potentially poisonous substances were inaccessible to the children in care. Evidence: There was a leaf blower sitting on a table (about 3' high) located on the children's playground. The leaf blower had a container of gasoline attached to it. The playground was not in use at the time of the inspection. Plan of Correction: Corrected during the inspection.
