Evidence: Household member 2 and Household member 4 did not have documentation of a current tuberculosis test or screening. Plan of Correction: Household members will get documentation of a current tuberculosis test or screening.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of three household files and two staff files, the provider failed to have repeat sworn disclosure statement for each household member. Household member 2 did not have repeat sworn disclosure statement. Plan of Correction: Household member 2 will complete a repeat sworn disclosure statement.
Standard #: 22VAC40-191-40-D-4-a Description: Based on review of four household files and two staff files, the provider failed to have documentation of background checks on file for each person living in the home of the licensed family day home. Evidence: Household member 4 moved into the house 12/31/2017 and does not have documentation of a sworn disclosure statement, criminal history record or fingerprinting, or search of the central registry (CPS check). Plan of Correction: Provider will obtain background checks for Household member 4.
