Please complete http://www.dss.virginia.gov/facility/injury_report_subsidy.cgi to report any serious injury to the Division. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment. Please label all substitute containers containing hazardous substances with the contents (for example, bleach and water in a spray bottle).
An unannounced subsidy health and safety inspection (SHSI) was conducted with two assistants. A licensing inspection was also conducted on this date (see separate report for details). There are currently 3 assistants working at the FDH. The provider and all assistants need to complete the mandatory VA Pre-service Training. All staff are currently certified in first aid and CPR . There are no children with diagnosed food allergies currently enrolled ? please review the applicable standards regarding food allergies. The emergency preparedness plan shall include reunification plans, home containment procedures, and lockdown procedures. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children from a security threat; shelter in place drills are to protect children from a natural or manmade threat. If you have any questions regarding this inspection, please contact me at email@example.com. If you have specific questions regarding the subsidy program, visit our website at http://www.dss.virginia.gov/family/cc/assistance_providers.cgi Time of today?s inspection: 2:00 a.m. to 4:00 p.m.

In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.Virginia Quality is a voluntary quality rating and improvement system for early care and education facilities serving children ages birth through pre-K. Eligible child care facilities must be fully licensed, licensed exempt and a VDSS subsidy vendor, or a voluntary registered day home and a VDSS subsidy vendor.
