Comments:
A Subsidy Health and Safety inspection was conducted on 06/12/2018 from 10:11 a.m. to 1:15 p.m to review supplemental health and safety requirements. The children were playing with toys, watching television, and infants were sleeping upon the inspector?s arrival. Today, there were five children, totaling 14 points, present with two caregivers. The caregivers were observed interacting with the children. Interviews were held with the caregivers throughout the inspection. Additionally, when appropriate the inspector interacted with children. The provider does not administer medication. The provider does not provide transportation. The provider does not have pets. Five children?s records, three caregiver/household member records and one household member's records were reviewed during the inspection. Last emergency drill: 05/29/2018 Last shelter-in-place drill: 05/29/2018 A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Three violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: VENDHOM-000-005-B
Description: Based on record review, the facility did not maintain a hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The daily attendance record only documented the child's name and date of attendance. The time of each child's arrival and departure was not recorded.

Plan of Correction: We will start logging the time.

Standard #: VENDHOM-000-029-B-5
Description: Based on review of the facility's emergency preparedness plan, the home did not ensure that the emergency plan contained lockdown procedures. Evidence: The facility's emergency plan was reviewed and did not contain lockdown procedures.

Plan of Correction: We will complete procedures by Friday.

Standard #: VENDHOM-000-030-A-3
Description: Based on review of documentation and interview, the facility did not practice lockdown procedures at least annually. Evidence: The facility did not have documentation that a lockdown drill was practiced. The provider confirmed that a lockdown drill was not completed.

Plan of Correction: We will conduct a lockdown drill by the end of the month.
