Discussed updating licensed hours of operation.

Comments:
A complaint investigation was initiated and completed on June 28, 2018 regarding capacity. Interviews were conducted, and documentation was reviewed. There was not a preponderance of evidence to support the allegation regarding capacity. However, six non-related violations are identified in the Violation Notice. Description: Based on observation, the family day home did not have a written record of children in attendance each day. Evidence: During the time of inspection, the most recent documentation of children in attendance was dated January 2018.

Complaint related: No
Description: Based on record review, one child's record did not contain all required information. Evidence: Child #1's (Start date: 6/28/2018) record did not contain documentation of immunization records for the child as required by 22 VAC 40-111-90.

Description: Based on observation, 2 staff members did not maintain documentation of current CPR certification. Evidence: Staff A and Staff B's documented CPR certifications expired 1/20/2018.

Description: Based on staff interview, the provider did not obtain a current report of tuberculosis screening for each household member prior to coming into contact with children. Evidence: During the time of inspection, there was no documentation of a current TB screening for Household member A, who moved into the residence February 2018.

Description: Based on staff interview, a household member over the age of 18 years did not have documentation of a sworn statement, central registry, or criminal history background check. Evidence: During the time of inspection, there was no documentation of a completed sworn disclosure, central registry, and criminal history background check for Household member A, who moved into the residence February 2018.
