An unannounced renewal inspection was conducted today at the Family Day Home from 10:40am-12:50pm. A total of 3 children [7 points] were present with one caregiver. Children were observed during arts and crafts, handwashing, snack, chores and while singing nursery rhymes. Nurturing and positive interactions were observed between children in care and provider. A sample of 3 children's records and 6 caregiver/household member records were reviewed, in addition to emergency drills, emergency plan, emergency supplies, physical plant and other documents. Standard #: 22VAC40-191-40-D-4-a
Description: Based on a review of records, 1 of 5 household member records was missing a sworn statement when a person who lives in the home became 18 years old. Evidence: The record for Household member #2 [18 years old, as of June 2018] was missing documentation of a completed sworn statement.

Plan of Correction: I will have the household member complete the document today.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of records, 1 of 5 household member records were missing a child protective services central registry check within 30 days of becoming 14 years old. Evidence: The record for household member #4 [14 years old, as of March 2018] was missing results of a central registry check.

Plan of Correction: We will fill the form out immediately and send it off.
