Reviewed appropriate diaper changing procedures.
An unannounced full renewal inspection was conducted today. The provider was caring for 5 children, totaling 16 points. Children's records, adult/caregiver records, background checks, emergency preparedness items, the indoor and outdoor physical plant, children's nutrition, and children's programming were inspected today. The children were observed enjoying outdoor water play activities, reading books, listening to music, and playing with toys. The children were to be served pizza, corn, sliced grapes, and milk for lunch today. This inspection was completed between approximately 10:15am and 11:35am. Thank you for meeting with me today. Standard #: 22VAC40-111-830-A Description: Based on a review of emergency preparedness items, it was determined that the provider has not conducted an emergency evacuation drill each month. Evidence: The provider did not conduct an evacuation drill during the month of June, 2018. Plan of Correction: I will do two drills this month and then remember every month.
Standard #: 22VAC40-111-100-A Description: Based on a review of children's health records, it was determined that the children's records are not complete. Evidence: Child records #1, #2, and #5 do not include a physical exam report. Each of these children have been enrolled for more than 30 days. Plan of Correction: I will ask each parent to bring the physical form.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that four records were not complete. Evidence: 1. Child record #1 does not include: the mother's work contact information; the address for each emergency contact person; a signed medication policy document. This record also does not include written permission from the parent for the child to participate in swimming/wading activities. Upon arrival, the Inspector observed this child playing in the backyard wading pool. 2. Child records #3 and #4 do not include: the parent's signature authorizing the provider to obtain emergency medical treatment; a signed document indicating the provider's liability insurance. 3. Child record #5 does not include the address for each emergency contact person. Plan of Correction: The parents will be asked for the missing information.
