The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 7/10/2018. The renewal of the family day home license will not be processed until the background check results have been verified by the licensing inspector.
An unannounced renewal inspection was conducted today from 12:00pm-1:00pm. There were 8 children including a family member (21 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today in accordance with the family day home center's policy. Children were observed playing freely after finishing lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child 4's record did not contain a record of a physical examination by or under the direction of a physician prior. Child 4 has been enrolled at the center for a period of over 30 days. Plan of Correction: I will ask the parents to provide a physical.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations for one of the four children's record reviewed. Evidence: Child 4's record did not contain a record of immunization. Plan of Correction: I will ask the parents to bring the immunization record.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain at least once every six months a record of immunization for children under the age of two. Evidence: Child 2's record's most recent immunization was documented on 8/22/2017. Plan of Correction: I will ask the parents to bring updated immunizations.
Standard #: 63.2-1721.1-B-2 Description: Based on record review, the provider did not obtain criminal history record reports for household members over the age of 18. Evidence: Adult 3's record did not contain the results of a national fingerprinting background check. Plan of Correction: The household member's background check has been scheduled for next week. The results will be shared with the licensing inspector when they arrive.
