Comments:
An unannounced monitoring inspection was completed on 7/11/18. There were nine children present with the provider and an assistant supervising. The point total was 21. Five newly enrolled children's records were reviewed and 11 children's records were reviewed to ensure annual updates were completed. The provider's record, one household member's record, and three assistant caregiver's records were reviewed for updates (one new hire caregiver's entire record). No medications were being administered. The inspector arrived at the provider's home at 12:00 pm and departed at 3:00 pm. The children were observed preparing for nap and during nap. Arrival and departure of children were also observed. There was discussion with the provider about the following: changes made to the child care area to include new additions for one level child care and a separate preschool classroom, background check changes, upcoming trainings, new forms on the web site for subsidy providers.

Description: Based on record review and interview, the provider failed to ensure that one newly hired substitute's record contained all required information. Evidence: 1. The record of one assistant caregiver was missing the following required information: verification of age, date of employment, name, address, and telephone number of a person to notify in the event of an emergency, two or more references as to character and reputation as well as competency that were checked before employment. 2. According to the provider, the assistant had been working since either June 18 or 19, 2018.

Plan of Correction: The provider will have the assistant bring letters of reference and complete the provider record form. The provider will copy the assistant's identification card for verification of age.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and interview with the provider, the provider failed to ensure that each employee had a fingerprint record completed as a part of the background check prior to employment. Evidence: 1. One assistant caregiver had a completed fingerprint eligibility letter dated 6/26/18. According to the provider, the assistant began working either June 18 or June 19, 2018. There was no fingerprint check completed prior to the assistant's first day of working.

Plan of Correction: The provider will ensure that any future caregivers have a completed fingerprint eligibility letter prior to beginning work.
