Discussion that the FDH has obtain the fingerprint background checks for all staff and applicable household members. As a reminder, new employees will not be allowed to start until an ?Eligibility Letter? is on file.
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located near Lovettsville, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (17 points) with 3 caregivers. A total of 5 children?s files, 4 staff files, and 4 household member?s files were reviewed. The children were engaged in the following: large motor skills outside on playground, snack, hand washing, diapering, story time, toy play, learning, lunch, and other organized activities. Standard #: 63.2-1721.1-A Description: Based on review, 1 of 4 staff files reviewed indicated that the Family Day Home (FDH) did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him or her. Evidence - 1. On the date of inspection (07/10/2018), an out-of-state CPS check from the State of Maryland was not available for review for Staff #3. 2. Based on a completed CPS background check for the Commonwealth of Virginia, Staff #3 listed that she resided in Maryland in 2014. Staff #3 also indicated this on a completed Sworn Disclosure Statement dated - 07/14/2017 where Maryland was listed. Plan of Correction: An out-of-state CPS check from Maryland shall be sent for Staff #3.
