63.2 Licensure and Registration Procedures
Today an unannounced monitoring visit was conducted with the Provider. This visit took place in the morning between 9:15am-11:30 there were 8 children with the Provider and 2 assistants for a total of 20 points within the supervision guidelines. The children were observed having outdoor free play, swinging, playing with blocks, learning colors, alphabets and letters, while in transition for lunch and nap time. Positive interactions were observed during this inspection. Lunch consisted of ham sandwhiches, fruit, green beans and milk. A complete inspection of the physical plant, staff and children records, fire drill log, menu, and attendance records were observed. Standard #: 63.2-1720.1-B-3 Description: Based on interview with Provider and review of staff/household records, the provider did not obtain a Out-of-state Central Registry check for staff. Evidence: 1. Staff #1's Sworn Disclosure Statement confirmed that they currently live/have lived out-of-state. 2. Record for Staff#1 (hired 5/2018) did not have documentation of an out-of-state Central Registry check completed prior to employment. Plan of Correction: Provider stated that they will have the out of state check completed immediately.
