Technical Assistance:
Discussed updating Topical skin products every year. Discussed Central Registry Findings will be sent by email. Discussed Out of State Central Registry requirements.

Comments:
An unannounced monitoring visit was conducted today between the hours of 9:46am through 12:15pm with the provider on site. There were 5 children (18 points) with the provider and assistant. A sample of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: playing with Lego and infant toys. The home is clean and organized. Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, furnishings of the family day home inside were not maintained in a safe operable condition. Evidence: 1. Two white cribs had visible chipped paint on the crib railings.

Plan of Correction: We sanitize the crib everyday and caused the new cribs to chip. I will have the parent's return the crib for new ones.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface did not meet all requirements. Evidence: 1. The diaper changing pad on the changing table had several small visible rips on the pad and were not nonabsorbent and washable.

Plan of Correction: I have a plastic pad on top of it and did not see the rip, but will replace the pad.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the provider does not have a central registry finding within 30 days of employment for a caregiver. Evidence: 1. The Assistant began employment with the provider on 4/24/2018 and the provider did not have documentation of sending the central registry and did not have the findings within 30 days of employment.

Plan of Correction: I gave the paper to the Assistant to fill out today. I didn't realize I had to send it with the new fingerprint requirements.
