Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on July 19, 2018 from 11:00 a.m. through 1:00 p.m. There were ten children in the care of the provider and two assistants for points totaling eighteen. Five children's records were reviewed. The records for the provider, one household member and three assistants were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were free choice play, games, hand washing/diapering procedures, and nap. Interactions between the children and staff were observed as positive and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-111-180-A Description: Based on record review the provider failed to ensure an updated tuberculosis screening had been obtained for one household member. Evidence: 1. Records were reviewed. 2. There was no documentation of one household member obtaining an updated tuberculosis screening. 3. Last tuberculosis screening documented for Household member #1 was documented 04/11/2016. Plan of Correction: An updated tuberculosis screening will be obtained.
