An unannounced monitoring inspection was conducted on Friday, July 13, 2018. Present today were two children totaling four points with the provider. The provider and the children were preparing to leave to go swimming. Lunch plans were to eat at a local restaurant . A practice evacuation drill was conduced 6/12/18. A practice shelter-in-place drill was conducted 4/19/18. The provider is currently certified in first aid and CPR. All adult records were checked and all fingerprint searches had been completed and filed.
Standard #: 22VAC40-111-180-A Description: Based on review of records, the provider did not ensure all individuals requiring an updated Report of Tuberculosis (TB) Screening form was competed every two years from the date of the last screening. Evidence: The provider TB expired 6/20/18. Plan of Correction: Per Provider: "Will fix within next two weeks."
